What is uterine synechia: Our treatment

It is a condition where the two sides of the uterus stick together, normally the uterus functions as a hollow cavity lined with a mucous membrane (a sort of carpet that covers it) which prevents one side from sticking to the other: it's like an empty glove that, when you insert fingers, allows itself to be stretched. If the lining deteriorates and disappears, the two sides of the cavity can stick together, which can reduce the size of the cavity and potentially hinder implantation and the development of a pregnancy. These adhesions can vary in size (surface area) and therefore have equally variable consequences. In the case of very extensive forms, they can even affect the volume of menstruation, which may then be absent.

What are the causes?

It is generally a matter of a follow-up to a miscarriage (natural or induced), whether a curettage was performed or not. This is not necessarily the direct result of this curettage, but rather the most likely change in the lining caused by the pregnancy or the retention of residues from it. It can also concern post-partum follow-up (sometimes after a cesarean section), especially after an infectious episode. It is also possible, but more rarely, that it is following uterine surgery (especially after myomectomy).

Thus, this scenario can occur either due to a change in the lining after a procedure on the uterus, or as a result of an infection (particularly severe cases post-genital tuberculosis should be mentioned).

The symptoms of a synechia

The symptoms of uterine synechia vary depending on the severity of the adhesions. Some women may not feel anything, but the most common signs are:

Main symptoms

  1. Menstrual disorders: Light periods (hypomenorrhea)
  2. Absence of periods (secondary amenorrhea)
  3. Pain during menstruation if the blood is blocked (dysmenorrhea)
  4. Infertility: difficulty conceiving despite regular intercourse.
  5. Repeated miscarriages: inability for the embryo to properly implant.
  6. Chronic pelvic pain (less frequent).

Rarer symptoms

  • Absence of blood despite premenstrual signs (a syndrome similar to "blocked periods").
  • Decrease in the quality of the uterine lining, leading to difficult or early-stopped pregnancies.

How do we do the diagnosis

Generally, the diagnosis is made thru a hysterosalpingography, performed in the context of an infertility evaluation: this then reveals a more or less extensive adhesion of the faces.

It sometimes happens that the diagnosis is mentioned due to the absence of menstruation following a surgical operation (such as a curettage, childbirth, cesarean section, or myomectomy), or a significant reduction in them: this is then referred to as amenorrhea (absence of menstruation) or oligomenorrhea (reduction of menstrual flow).

In case of suspected synechia, a comprehensive evaluation must be conducted, including: a hysterosalpingography, a diagnostic hysteroscopy, and a pelvic ultrasound. This will allow for the assessment of the severity of the synechia, the identification of any associated lesions, and the suggestion of the appropriate therapeutic option.
