What is syphilis?
Syphilis is a highly contagious sexually transmitted infectious disease (STI) caused by a bacterium from the spirochete family, known as Treponema pallidum or pale treponeme. Unprotected intimate contact with someone already carrying syphilis greatly facilitates its transmission. This transmission can occur during various sexual acts (anal, vaginal, and oral-genital).
The treponeme being unable to survive in the open air, contamination can only occur thru direct contact with:
- an isolated ulcer called a chancre, which is a characteristic symptom of primary syphilis superficially eroded skin or mucous membrane
- lesions (excoriated mucosa) present in cases of secondary syphilis.
Transmission of syphilis during pregnancy
Congenital syphilis remains uncommon, largely due to regular syphilis screening programs for pregnant women. Nevertheless, if a mother has syphilis and this disease remains untreated, she risks transmitting it to her unborn child. Transmission of bacterial syphilis via the placenta can begin as early as the 11th week of amenorrhea and is considered certain from the 16th week onward, continuing throughout the duration of the pregnancy. Syphilis can also be transmitted during childbirth, thru contact with infected maternal fluids.
What are the different stages of syphilis?
In untreated sick individuals, the progression of syphilis is marked by a series of clinical stages over time. We can break down the progression of the disease into two main stages:
- A first phase called early syphilis, which includes the first stage of the disease (primary syphilis) and the second stage (secondary syphilis).
- Latent phases, between the stages of primary and secondary syphilis, during which no symptoms are detectable. These periods coincide with stages where syphilis serology is positive, despite the absence of symptoms in the individual: this is referred to as early serological syphilis.
- We speak of late, or secondary, syphilis after a year of the disease's progression, which is characterized by tertiary syphilis or late serological syphilis. This advanced form of syphilis can only be identified thru a blood test.
What are the symptoms of syphilis? Manifestations that differ according to the stage of the disease
The symptoms of syphilis vary depending on the stage of the disease.
The syphilitic chancre, a characteristic symptom of primary syphilis
Syphilis is a sexually transmitted disease with an incubation period, meaning the time between contact with the bacteria and the emergence of the first signs, ranging from 10 to 90 days for the initial stage. At this stage, the infection reveals itself thru the emergence of a syphilitic chancre, also known as a syphilitic ulcer. This specific, usually singular lesion has a diameter of 5 to 15 mm and feels firm to the touch, which suggests a case of primary syphilis.
The chancre is superficial, clean with a pink base, and unlike herpetic lesions, it is painless. It is a major source of contagion because it contains a large amount of treponemes, the bacteria responsible for the disease. This chancre heals spontaneously in 2 to 6 weeks without leaving a scar and is often accompanied by swelling of nearby lymph nodes.
In humans, the chancre generally appears on the glans, in the area that connects the glans to the rest of the penis, or less frequently on the foreskin. In women, it is often located on the major or minor labia of the vulva. However, the diagnosis can be delayed if the ulcer is located inside the vagina or on the cervix, due to its painless nature. Regardless of gender, the chancre can also appear in the rectum, mouth, or throat.
Secondary syphilis: skin rashes and lesions
Secondary syphilis generally manifests between 6 and 16 weeks after the initial infection if it has not been treated. This stage can last for less than a year and is marked by the spread of the treponeme throughout the body, alternating between skin manifestations and asymptomatic phases. The manifestations include skin and mucous membrane eruptions, which have a high capacity for contagion.
The eruptions can vary in appearance, including:
- A syphilitic roseola, with small pink spots on the trunk, disappearing in less than 10 days.
- Papular syphilids, which are raised, copper-colored bumps on the face, limbs, trunk, soles of the feet, palms of the hands, and genitals. These lesions are contagious, especially when they are ulcerated and do not cause itching.
- Other lesions can mimic acne, seborrheic dermatitis, or manifest as patchy alopecia and perleches at the corners of the mouth, making diagnosis difficult.
In addition to skin lesions, secondary syphilis can be accompanied by general symptoms such as fever, fatigue, joint and muscle pain. It can also be complicated by the onset of meningitis.
