What is Aspermia? Causes, types, and treatment

Aspermia is the complete absence of sperm (not to be confused with azoospermia, the lack of sperm in semen). Naturally, it is associated with infertility.

What is anejaculation?

Ejaculation refers to the expulsion of 2 to 5 ml of semen thru the urethral meatus. This reflex expulsion occurs in men after puberty and is accompanied by an orgasm. It responds to a mechanism involving the nervous system, the hormonal system, and numerous glandular, canalicular, smooth or striated muscular structures. It occurs following a chain reaction progressing in this order: birth of desire, achievement of an erection, occurrence of orgasm, expulsion of sperm thru the urethra. This cascade of events, more or less controllable, unfolds in two stages:

  • emission phase resulting from penis stimulation with the triggering of secretion by the seminal glands and the prostate. The seminal fluid, composed of spermatozoa (produced in the testicle) and seminal fluid, accumulates in the posterior urethra;
  • a phase of sperm expulsion thru the urethral meatus thanks to the opening of the striated urethral sphincter, facilitated by the contraction of the perineal muscles.

This physiological manifestation is orchestrated by two nerve centers located in the spinal cord and the brain.

This disorder can affect male fertility and sometimes be a sign of a medical problem.

The types of aspermia

We distinguish two forms:
✔ True aspermia
The man does not ejaculate any liquid despite stimulation or orgasm.
✔ Aspermia with retrograde ejaculation
The sperm exists, but it goes back into the bladder instead of coming out thru the urethra.
Patients may notice cloudy urine after sexual intercourse.

Causes of aspermia

The causes can be:

a) Psychological causes

  • Stress
  • Performance anxiety
  • Depression
  • Sexual traumas

b) Neurological causes

The nerves that control ejaculation can be affected:

  • Poorly controlled diabetes
  • Spinal cord injuries

Symptoms of aspermia

The main sign is the absence of ejaculation, but other manifestations may appear:

  • Orgasm without sperm expulsion
  • Infertility
  • Cloudy urine after intercourse (if retrograde ejaculation)
  • Pain during orgasm (rare)
  • No sensation of ejaculation

Diagnostic

The doctor can perform:

✔ Clinical examination
Study of the history, genital examinations, medication review.
✔ Post-orgasmic urine analysis
To check for the presence of sperm in the bladder (retrograde ejaculation).
✔ Spermogram
To confirm the total absence of sperm.
✔ Transrectal ultrasound
To detect an obstruction of the ejaculatory ducts or a malformation.
✔ Hormonal tests
Measurement of testosterone, FSH, LH, prolactin.
✔ Neurological assessment
To check the condition of the nerves and spinal cord.
