What is ovarian dystrophy?

Ovarian dystrophy is a hormonal abnormality. With an estimated prevalence of between 4 and 12%, it is the most common endocrine pathology in women of childbearing age.

It is characterized by an abnormal process during the maturation of the follicles, during the first phase of the ovarian cycle (the follicular phase, just before ovulation). During the last stage of follicular growth, many small follicles (25 to 30, compared to ten normally) accumulate without starting to grow. This is reflected on ultrasound by the accumulation of multiple small cysts – small ovarian follicles in reality – around the ovaries, hence the name polycystic ovary syndrome (PCOS), another name given to this pathology.

Diagnosis of ovarian dystrophy

Among the symptoms of ovarian dystrophy we have:

irregular periods,

infrequent or totally absent (amenorrhea) or on the contrary,

menorrhagia (very heavy periods),

hirsutism (hypervelocity on the face, neck, etc.) due to high secretion of testosterone, acne,


acanthosis nigricans (browning and thickening of the skin on the neck,

groin, armpits and skinfold).

These signs are not necessarily all present, and this clinical heterogeneity makes the diagnosis of ovarian dystrophy sometimes difficult.
clinical (hirsutism, acne, androgenic alopecia) or biological hyperandrogenism;
oligo-anovulation (irregular or absent ovulation). According to the Rotterdam criteria, cycles shorter than 21 days or longer than 35 days are considered anovulatory;
the morphological appearance of the ovaries on endovaginal ultrasound.
Obesity can be associated with ovarian dystrophy, however, it is not known with certainty whether obesity causes this pathology or, on the contrary, if it is what leads to overweight. The pathophysiological mechanisms at the origin of ovarian dystrophy have not yet been elucidated. It is assumed that at the base, hyperandrogenism, that is, excessive secretion of androgens (male hormones) affects the functioning of the ovaries. The cause could be genetic, and many studies indicate that PCOS results from primary ovarian abnormalities. But there are likely multiple mechanisms that interact with each other.

Can ovarian dystrophy prevent having a baby?

It varies from woman to woman. Whether the woman is ovulating or not, both cases exist in the case of ovarian dystrophy.

If the ovarian follicles accumulate without any maturing, there can be no ovulation and therefore fertilization. Ovarian dystrophy is thus the cause of more than 70% of infertility by anovulation. Ovarian dystrophy is also sometimes diagnosed during the infertility assessment.

In some women, some follicles continue to mature and the dominant follicle reaches ovulation. This sometimes happens irregularly.

Note that being overweight can aggravate the clinical features of PCOS, including subfertility. In overweight or obese women, weight loss is sometimes enough to restore quality ovulation.

What is the treatment for infertility?

No treatment can cure ovarian dystrophy.

In case of subfertility due to ovarian dystrophy, the treatment will aim to restore quality ovulation.

Ovulation induction with clomiphene citrate is the first-line treatment. This anti-estrogen promotes the maturation of the follicles. It achieves pregnancy in 35-40% of patients with PCOS (6).

In the event of failure, other hormonal stimulation protocols may be attempted: a combination of metformin and clomiphene citrate, gonadotropins or aromatase inhibitors.

Ovarian drilling is sometimes performed. This is a small surgical procedure consisting of laparoscopically performing a “multiperforation” of the ovary in order to restore its proper functioning.

In vitro fertilization sometimes preceded by in vitro oocyte maturation (IVM) (the oocytes are collected before ovulation, at a late stage of follicular maturation, and completion of their in vitro maturation) may be performed as a last resort, or when male fertility is also impaired.

1) Let's eat well, let's eat better!

The first thing you need to know is that your diet plays an important role in treating the various symptoms of PCOS.

Again, in order to best adapt your diet to your situation, we recommend that you consult your doctor, as well as a nutritionist/dietician before implementing any change in eating habits.

However, here are some tips we can give you on how to eat with PCOS:

Choose fresh, whole foods, etc. And avoid frozen or cooked meals.

- Eat vegetables during each meal, and vary your vegetable colours!

- Eat “Good Fats”! (Avocado, Coconut Oil, Olive Oil, etc.)

- Healthy snacks for your small appetites! ;) (Nuts for example!)

- “Flee” sugar, white flour, sodas and industrial fruit juices.

- Limit grain products. (Pasta, bread, rice, etc.)

- Be careful with alcohol (relatively high glycemic index!)

- Reduce dairy products.

- Control the composition of the products you consume (controversial ingredients, etc.)

A healthy diet does not dispense with regular physical activity.

2) Acne is ugly!

Mint infusion: Mint has anti-androgenic properties. It is able to reduce the level of testosterone and therefore limit the production of sebum.

How to prepare it? Pour boiling water over a heaping teaspoon of dried or fresh mint leaves (about 5 grams) and steep for 5 to 10 minutes. 2 cups per day are recommended.

We recommend drinking this infusion at least 1 hour apart from your meals (before or after) to allow optimal absorption of the iron provided by your meals.

Clay: It acts against the bacteria responsible for acne pimples: Propionibacterium acnes

Mix a small amount of clay powder with water. Apply the mixture generously to the area to be treated. When the clay is dry, then rinse with clear water.

3) Blood sugar:

Cinnamon: It reduces the level of glucose in the blood and is therefore hypoglycemic, it also has an action on insulin resistance. When cinnamon is added to another food, it lowers the glycemic index of that food. It also has antioxidant properties.
