Annexitis: what is it?
Annexitis is a medical condition characterized by inflammation of the area adjacent to the uterus. More specifically, it involves the fallopian tubes and ovaries, which surround the uterus. The origin of this pathology is a bacterial infection. When the inflammatory process extends to other parts of the female reproductive system, it is referred to as pelvic inflammatory disease.
Annexitis is a serious gynecological disease that can damage the fallopian tubes and ovaries and even lead to the patient's infertility. Moreover, one can encounter bilateral annexitis, which will affect both tubes and hinder a natural pregnancy.
Causes of annexitis
The cause of appendicitis is generally a bacterial infection. This, in turn, can stem from various origins:
- Sexually transmitted infections, such as chlamydia or gonorrhea.
- Bacterial infections that ascend from the lower genital tract, including the vagina and cervix.
There are also other bacteria such as staphylococci, streptococci, enterococci, or mycoplasmas and anaerobes, which can also trigger an adnexitis. The viral origin of this pathology is less frequent.
Risk factors for contracting an infection
The highest risk of contracting an infection leading to annexitis is in women of reproductive age. Generally, they are between 16 and 24 years old and frequently change sexual partners. We are talking about women who were very young when they had their first sexual encounter and/or who had their first encounter with a partner known to have a sexually transmitted disease.
It is rare to contract this disease during puberty or at menopause.
Regarding the routes of infection, they are of two types.
Ascending infection
The pathogens travel from the outside thru the vagina, toward the fallopian tubes and ovaries. It is almost exclusively sexually mature and active women who contract the disease thru this route of infection, for the following reasons:
- Births and miscarriages
- Sexual intercourse
- Gynecological procedures such as IUD insertion
- Presence of benign tumors such as fibroids or polyps in the cervical region.
Hematogenous infection
It occurs when the pathogen reaches the fallopian tubes and ovaries thru the bloodstream. This would be the case with Mycobacterium tuberculosis.
Annexitis: symptoms
There are many symptoms associated with annexitis. Here are some of the most frequent ones:
- Abdominal or pelvic pain
- Fever and cold sweats
- Pain during sexual intercourse (dyspareunia)
- Pain or difficulty urinating (dysuria)
- Irregular menstrual bleeding
Acute annexitis
If these symptoms occur with high intensity, it is a case of acute annexitis. It will manifest as severe abdominal pain, shortly before or during menstrual bleeding, which will increase with sexual intercourse. Moreover, there will be abdominal distension along with abdominal wall tension.
If the annexitis is acute, the infection will cause fever and more pronounced symptoms:
- Pain when urinating
- Intermittent bleeding
- Nausea
- Diarrhea Enlarged uterus, sensitive to pressure
Chronic annexitis
When acute annexitis is not properly treated, the symptoms persist or reappear and the disease becomes chronic. It can also be the result of scar tissue changes in the tissues of the reproductive organs, derived from a previous inflammatory process. The symptoms can last for months or years, some of them being swelling, pain, or irregular menstrual periods.
